Our goal is for your patients to return to your office as soon as possible after oral surgery, so they can continue their routine care. We are in-network with many dental insurances and provide affordable care. Our office is open five days a week and available for emergencies. You may refer patients to our office by filling out our secure online Referral Form. We can also provide paper referral forms upon request. Please be as detailed as possible with your referral, with tooth number, type of treatment requested, and any pertinent history. Email any x rays to [email protected]. We would like to minimize any phone calls and interruptions to your office. Please ask your patient to call our office at 408-298-7775 for a consultation appointment. If you prefer, we can also reach out to your patient.
